That new rule starts in 2020. Already there's speculation that pitchers may fake an injury to facilitate a pitching change and solutions to that are, of course, heavy on the punishment.

Section II, a.
1.Exceptions a.:
If the offended player is injured, other than as a result of a flagrant foul or unsportsmanlike conduct, or is ejected from the game and cannot attempt the awarded free throw(s), the opposing coach shall select, from his opponent’s bench, the replacement player.

So, if a pitcher cannot continue, the opposing manager selects the relief pitcher to replace the injured pitcher.
